New Immunization Requirements for grades 6-12 from Illinois Department of Public Health (IDPH) and Illinois State Board of Education (ISBE) beginning January, 2012:
- Beginning January 2012, any child entering 6th and 9th grade shall show proof of receiving one dose of Tdap (tetanus, diphtheria, acellular pertussis) vaccine regardless of the interval since the last DTap, DT or Td does.
- Students entering grades 7-8 and 10-12 who have not already received Tdap are being encouraged to receive 1 Tdap dose regardless of the interval since the last DTap, DT or Td dose.
Whooping Cough, also known as pertussis, continues to occur in Illinois. In an effort to protect children and adolescents from the serious complications associated with this disease, all students entering grades 6 and 9 will be required to present proof of vaccination. Please make an appointment with your health care provider now to receive this important vaccination or to verify that your child has received this booster. Questions or concerns can be addressed to your Certified School Nurse.
ATTENTION Parents of 2012-2013 Incoming 6th Graders
Please be aware that the State of Illinois has changed the immunization requirements for student's entering 6th grade. Rules of the Illinois Department of Public Health Child Health Examination Code (PART 665) states:
"Beginning with school year 2011-12, any child entering sixth grade shall show proof (see Section 665.250(b) of receiving one dose of Tdap (defined as tetanus, diphtheria, acellular pertussis) vaccine regardless of the interval since the last DTaP, DT or Td dose."
Please check with your physician to make sure your child is receiving or has received the Tdap vaccination.
Physical Forms & Immunizations for Early Childhood, Pre-K, K, 6th and 9th Grades

Child Health Examination forms and required immunizations MUST be submitted prior to the first day of school in order for students in K, 6th and 9th grades to begin school.

Early Childhood (including Walk-in Speech students) & Pre-K students must submit required health records and immunizations prior to the start of services.

The physical exam must be completed within one calendar year prior to the start of school and must fulfill all Illinois State Board of Education and Illinois Department of Public Health requirements.

Students will not be allowed to begin school without a submitted physical examination form and required immunizations.
- Students who are new to District 303 and IL schools must submit health records within 30 days of entering school. Transferred health records will be reviewed by the school nurse for compliance with Illinois school health requirements.
Dental Forms
- Illinois State law now mandates that all K, 2 and 6 grade students present proof of a dental examination prior to May 15 of the school year.
- A waiver has been established by the Illinois Department of Public Health for students who show undue burden or lack of access to a dentist.
Vision Examination Forms
- Illinois State law now mandates that all incoming K and students new to Illinois Schools must present an eye exam report conducted by a physician who provides a complete eye exam or a licensed optometrist.
- This report must be submitted to the school by October 15 of the school year and must be completed within one year prior to the required date.
- A waiver has been established by the Illinois Department of Public Health for students who show undue burden or lack of access to an ophthalmologist or optometrist.

Students requiring rescue medications
To self-carry/self-administer a quick acting inhaler
Life-Threatening food allergy requiring epinephrine and/or Benadryl
Life-Threatening non-food allergy requiring epinephrine and/or Benadryl
To self-carry/self-administer Epinephrine and/or Benadryl
All other medications at school (prescription and over-the-counter)
